Rebate Program - Baby's Only Organic Formula

For every 24 proofs of purchase saved from one of our formula products, we will send a coupon for 2 free units of Baby’s Only Organic® formula through mail-in rebate. That is our way of saying "thanks" for buying and supporting our products.

Or, instead of receiving free formula, you may pledge your product to Feed the Children, a non-profit agency that helps feed hungry children all over the world.

Nature's One® will simply add your individual donations to the amount given each year from our profits. See Feed the Children's web site at www.feedthechildren.org for more information on how you can participate directly in their life changing programs.

The "proof of purchase seal" and the product's numbered UPC bar code are the same. The UPC code is located on the back of the label with the text "proof of purchase" printed above the black bar codes. Carefully cut the UPC bar code from the back of the label. Be sure to include all the numbers and bar codes of the UPC to insure verification.

You can access Nature's One's on-line rebate form at this link: http://www.naturesone.com/rebate-form.php. Below are instructions for completing the rebate process:

1. Accumulate 24 UPC labels (UPC barcodes)
2. Complete the Mail-In Rebate Form
3. Print out the Mail-In Rebate Form
4. Write your most recent Nature's One Direct Order Number on the Mail-in Rebate Form
5. Staple or attach Proof of Purchase labels to the Mail-In Rebate Form and mail to:

Please allow 3-4 weeks for delivery after receipt of proofs-of-purchase.

• No copies or duplicated Proof of Purchase Seals will be accepted.
• Lost or stolen requests cannot be replaced, verified or honored by Nature's One®.
• Offer void where prohibited. Nature's One reserves the right to withdraw or change offer without notice.

Indicate you are a Nature's One Direct customer and write your most recent order number on the Mail-In Rebate Form. Once we receive your rebate form with the 24 UPC labels (UPC Barcodes), we will credit your account for the value of two free cans.
